* QCR Holdings Inc. said that Mary Kay Bates is a new independent director of the Moline, Ill.-based company.

Bates currently serves as the president and CEO of Spirit Lake, Iowa-based Bank Midwest, a unit of Goodenow Bancorp.

* Midland, Mich.-based Chemical Bank named Brad Haverkamp community president and commercial group manager for the Lakeshore, Mich., area, the Grand Rapids (Mich.) Business Journal reported.

He most recently served as senior vice president at JPMorgan Chase & Co.

Chemical Bank is a unit of Chemical Financial Corp.

* Kokomo, Ind.-based Community First Bank of Indiana appointed Pete Smith to its board, the Kokomo Perspective reported.

Smith works as a consultant for an engineering company called Performance Services.

The bank is a unit of Community First Financial Corp.
