Hubert Grubbs, a 29-year-old miner, suffered fatal injuries at Revelation Energy LLC's D11 Panther Mine in Cumberland, Ky. and died on March 28. The incident marks the first mine-related death in the state and the fourth in the country.
Grubbs, a pre-shifter/belt examiner with 10 years of experience, sustained his injuries when the conveyer belt he was splicing started unexpectedly, the Kentucky Energy and Environment Cabinet said in a news release.
The Kentucky Department for Natural Resources, Division of Mine Safety, dispatched investigators to the mine on the same day of the incident to look into the matter. Mining operations on the site were halted and remain under suspension while the investigation continues.
The mine is in the process of a full inspection and has been issued 15 citations for noncompliance and one closure order, according to the release. The closure order and all but six noncompliance citations have been abated.
